SU ADVICE CLIENT INFORMATION FORMS
SU Advice operates an electronic database to hold a record of your query. We ask you to provide us with some information to allow us to keep accurate records. Please download and complete the following Client Information Form, using your name as the file name and once completed return it to us via email. Alternatively, you can complete a paper copy of the form by visiting SU Advice in person.

Your adviser may need to contact other University services or departments when seeking to assist you with your enquiry. In this case they will need to request you complete a 'Permission to Discuss' form. If you have been asked to provide your permission to discuss, please download and complete the attached form. Once complete, please save it with your name as the file name and return it to SU Advice via email.
